Victoria Voxxx packs her guitar and her ambition ("I'm gonna be a star") and hits the road to follow her dreams, landing in a rural small town and working at a beer bar for room and board. Alone in her room, she noodles on her guitar and tries out lyrics, writing them in a notebook. Neighbor and fellow music wannabe Sophia Locke tells Voxxx that laconic beer bar owner Chad Alva used to be a country music star and invites her to a jam. Meanwhile, neighbors Valentina Nappi and Isiah Maxwell hook up next door, which Voxxx hears through the thin wall between their apartments, distracting Voxxx as she tries to keep writing the song she started.

When Voxxx visits Alva at his place, he laconically hints at his past music career and they share family memories, her about her remote parents and him about his estranged child. The next day, over a bottle of wine, Locke tells Voxxx about the small-town gossip and details of her divorce ("I learned that I love to fuck, and he couldn’t give me the goods, so now I just take what I want when I can.") What Locke wants is Voxxx, and Locke's people-have-sex-differently pointed chat is interrupted by Queenie Sateen, guitar in hand, who encourages Voxxx to play the song she's working on, leading to the three sharing music—with Voxxx improvising harmony as Sateen sings—and then, after the sun goes down, each other.

When an expected trucker convoy changes course away from the town, five-years-sober Alva falls off the wagon, upset that he may lose the bar after the loss of business. Locke pep-talks him into coming back to performing to draw customers, getting an instant response moments after she announces it online. Alva asks old bandmate Charles Dera to perform with him, and he agrees ("I'll be there. You just say when and where.") before getting back to groupie action with Kendra Sunderland, August Skye and Nicole Doshi. Alva laconically asks Voxxx to be part of the show, and she readily accepts ("It'd be an honor. You're Rob Donahue, country music legend!") and, as a supporting act, Voxxx and Sateen do a star-is-born duet.

In the post-concert hubbub, Voxxx reunites with ex-boyfriend Roman Todd—who annoyingly calls her "Vicky" ("You know I hate when you call me that")—but she can't resist the guy and they hook up "one last time" for an impromptu orgy along with the rest of "the old gang": Ariel Demure, Dillon Diaz and Cliff Jensen.

When the cash from the concert doesn’t quite make the nut, Dera struts in and offers to bail out the bar and also manage Voxxx's career, making the beer bar a destination for country music fans. With the bar saved and Voxxx's country-music star dreams about to come true, Alva and Voxxx celebrate. Horizontally.

The measured pace of the production reflects the laid-back country music/small town elements of the story. Adding an element of realism, Voxxx, Sateen and, eventually, Alva do their own singing. Alive was nominated in five categories at the 2025 AVN Awards: Best Music, Best Leading Actress for Victoria Voxxx, Best Leading Actor for Chad Alva, Best Supporting Actress for Sophia Locke, and Grand Reel.
