When you see an Iron Maiden in some girl's frilly bedroom, it's not standing there to serve as a simple conversation piece. Before Mistress instructs her girls in its proper usage, they engage in playful spanking, nipple tweaking, hair pulling and, of course, the title fetish (by hand and paddle).

At one point, Anna is suspended by her arms while Madison enjoys the upright prison. Erzebet's gyroscopic sense of balance allows her to dig one heel in the former's crotch while flagellating the latter.

All of this action is appropriately arousing; unfortunately it would've been much more powerful if the Marquis knew what to do with a camera. His "point and shoot" style is downright sadistic - to the viewer! Wide shots are too wide. Close-ups are not composed so much as they're decomposed (for the least useful effect). Still, the title does what it needs to do, and then some.
